Social Worker (SW)- Part-Time/Per Diem (San Diego)

Job Description:

We are seeking an experienced and compassionate part-time Per Diem Social Worker to join our team. As a Social Worker, you will provide critical support to patients and their families, helping them navigate the emotional, financial, and social challenges of receiving care at home. You will work on a per diem basis, with flexible scheduling that suits your availability.

Responsibilities:

Conduct comprehensive psychosocial assessments of patients and families. Develop and implement care plans that address social, emotional, and environmental factors affecting the patient’s well-being. Provide counseling and support to patients and their families, helping them manage the challenges of chronic illness or injury. Assist in connecting patients to community resources, including financial assistance, housing support, and other social services. Advocate for patients' needs with healthcare providers, family members, and other stakeholders. Ensure accurate and timely documentation of services and progress in accordance with agency guidelines.

Qualifications:

Valid Social Worker license in the state of California (LCSW preferred). Minimum of 1 year of experience, preferably in home health, hospice, or a related field. Strong assessment, counseling, and communication skills. Ability to work independently and as part of an interdisciplinary team. Reliable transportation and willingness to travel to patient homes in Culver City and surrounding areas.
