The role you’ll contribute:
The Clinical Pharmacy Manager shall support the mission of the department, hospital, and health-system by preventing, identifying and resolving drug related problems to assure patient safety, improve patient satisfaction, optimize patient outcome and minimize cost. He/she will perform all functions of a registered pharmacist including accurate and timely dispensing, clinical interaction with the healthcare team, patient teaching, pharmacist and technician supervision.

Oversees appropriate utilization of medication automation, technology, and computer systems

• Prioritizes work load, organizes work flow, and checks accuracy of pharmacy technician and pharmacist activities
Pharmacy Practice
Completes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems
• Improper drug, dose, or dosage form selection
• Dose frequency or timing
• Subtherapeutic dosage
Proficiently verifies/enters medication orders and proactively communicates with medical staff to resolve problem medication orders
• Incorrect medication orders
• Ambiguous medication orders
Provides patientcentered care services
• Provides accurate, safe and timely medication therapy
• Initiates and facilitates appropriate therapy based on age, disease, and drug considerations
Initiates and facilitates appropriate drug dosing and manages medication-related pharmacist consults (as appropriate for assignments):
• Pharmacokinetic monitoring
• Renal dosing
• Nutritional support & TPN management
Facilitates medication safety practices
• Identifies, mitigates, and reports potential and actual adverse drug events, adverse drug reactions, and medication errors
• Documents pharmacist interventions and appropriately documents in the patient medical record
Maintains continuous learning and pharmacy practice development
• Maintains personal continuing education plan including documentation of an annual self-evaluation and plan for self-development
• Completes rigorous CE modules as part of state pharmacist licensure renewal (e.g. PSAP)
Leads and facilitates pharmacy practice programs, directs patient care services, and medication systems
• Works to develop metrics to document the provision of reliable, comprehensive, and individualized drug therapy that is aligned with patient needs, evidence-based best practices, regulatory standards, and budgetary targets
• Presents metrics to pharmacy leaders, medical staff leaders, and strategic business unit leaders regularly and as requested
Manage, coordinates, and oversees the pharmacy practice activities in a Clinical Pharmacy Division
• Clinical-Staff Pharmacists, Clinical Pharmacist, and Clinical Pharmacist Specialists providing pharmacy services within assigned patient services lines and therapeutic areas
Manages, coordinates, and oversees pharmacy care services and direct patient care programs in a Clinical Pharmacy Division
• Leads and manages all activities within assigned patient service lines and therapeutic areas
Collaborates with other Pharmacy Managers
• Develops measures to assure appropriate continuity of pharmacy services and patient care programs between services lines, therapeutic areas, and Clinical Divisions.
• Assures pharmacy staffing and practice coverage is consistent with patient care needs and medication use processes
Clinical Pharmacy Services
Provides primary coverage and management of direct patient care programs and multidisciplinary services
• Provides accurate, safe, timely, and appropriate medication therapy based on patient needs
• Places practice priority on the delivery of patient-centered care
Works as an active member of pharmacy clinical programs
• Collaborates with other pharmacy staff
• Collaborates with care providers and Pharmacists to optimize care and improve services
Provides general coverage of decentralized pharmacy practice areas
• Provides medication order verification in decentralized areas as assigned and supports other areas as needed
• Actively participates in clinical pharmacy programs and pharmacist consult programs (as defined in Pharmacy Practice Expectations)
Maintains proficiency to practice in decentralized areas of (at least one division):
• Cardiology Pharmacy Division
• Critical Care Pharmacy Division
Maintains proficiency to practice in (at least one areas, different than Division above):
• Cardiology decentralized units
• Critical Care decentralized units
Facilitates assigned patient service lines and therapeutic areas and assumes responsibility for the outcomes of the pharmacy care programs in assigned practice areas
• Oversees and facilitates the pharmaceutical care programs and activities within assigned services
• Assures pharmacy care programs are meeting the needs of patients
Maintains evidenced-based expertise in specialty pharmacy practice that is aligned with the needs of assigned clinical pharmacy service roles and patient care areas (expertise in the ASHP PGY2 standards for at least one area):
• Cardiology Pharmacy
• Critical Care Pharmacy
Manages, coordinates, and oversees all clinical services within Clinical Pharmacy Division
• Assures quality services
• Facilitates performance and continuing education plans for pharmacists
Qualifications
The expertise and experiences you'll need to succeed:
EDUCATION AND EXPERIENCE REQUIRED:
- Bachelor of Pharmacy, Doctor of Pharmacy, or other advanced pharmacy degree
EDUCATION AND EXPERIENCE PREFERRED:
§ PGY1 Pharmacy Residency OR Three years of clinical pharmacy experience
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
- Applicable State Pharmacy License
- BLS and ACLS
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
· BLS, ACLS, PALS and/or NRP may be required for some facilities based on department requirements and pharmacist role, as defined by specialty unit. Refer to hiring manager for needs.
