The Home health and Hospice Advocate is a critical role responsible for driving growth through strategic relationship-building and business development and who can leverage an existing network to generate referrals and promote Home health & Hospice Advocate Services. This position does not require experience but it does require compassion for the aging and sick adults who may or may not need hospice or home health.
Key Responsibilities:
-Develop and implement targeted marketing strategies to promote hospice services and increase referral volumes.
Establish and maintain strong, lasting relationships with referral sources, including healthcare providers, hospitals, skilled nursing facilities, and other community organizations.
-Act as a resource and subject matter expert to educate partners on hospice eligibility, benefits, and patient-centered care practices.
-Leverage an established network of contacts ("bank of business") to quickly build referral pipelines.
-Track and demonstrate measurable growth and effectiveness of outreach efforts.
-Represent yourself as a Home health and Hospice Advocate professionally at community events, health fairs, and industry networking opportunities.
-Collaborate with the clinical and administrative teams to streamline patient onboarding and ensure seamless transitions to care.
-Exceptional communication, negotiation, and relationship-management skills.
