ResponsibilitiesProvide intensive case management and home visitation to program participants and assist in program activities and administrative support.Implement identified evidence-based parenting education models with fidelityPlan and implement family relationship building activitiesPerform outreach and education activities within the communityFacilitate participant planning services to include interviewing, assessing, goal setting and referralsIdentify individual/family strengths that can be applied to overcome specific challengesDemonstrate knowledge of program objectives and scope of workComplete enrollment documents for each client referred by VCC and community partners into the programMaintain ongoing contact with clients to build trusting relationships, provide support, serve as a referral source and monitor progress toward identified goalsMaintain accurate, current participant files including attempted and actual contacts, flex funds, and document required program data and instrument as required by fundersDevelop and maintain a network of resources and services in the community to serve and assist with client needsSupport the vision, mission and goals, and demonstrate a commitment to the values, of the organizationPerform other duties as directedQualificationsMinimum
- Bachelor’s degree in related field (psychology, social work, etc.)
- Bilingual English/Spanish
- Minimum of one year education, case management or outreach experience
- Valid California driver’s license, current vehicle insurance and reliable transportation
- Availability and willingness to work evening and weekend hours as required
Required Skills/Knowledge/Abilities
- Experience/familiarity with computers, Microsoft Office products (Word/Excel), business e-mail communication systems and internet search capabilities
- Willingness to perform tasks outside the normal range of duties
- Ability to present information effectively verbally and in writing to individuals and groups from various cultural, ethnic and educational backgrounds
- Strong organizational, time management and interpersonal skills with ability to create a comfortable, supportive learning environment
