Job Description: Provides ongoing mental health and psychosocial client services. Services delivered include screening and assessing client needs and determining relevant treatment plans. Initiates or participates in community development initiatives, participating in PHC initiatives evaluation within site. Promotes quality health care outcomes working within established standards of practice to ensure that clients receive a high standard of primary health care. This will be done in accordance with organization mission statement, policies and procedures.
