SCOPE OF ROLE:
- The Social Worker will assist agency consumers with accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network, referral and follow up. The emphasis is on providing non-judgmental, non-coercive services and resources to individuals engaging in risky behaviors.
II. ESSENTIAL DUTIES & RESPONSIBILITIES:
- Actively monitors the integrity of consumer data; responsible for the reliability and accuracy of clinical data housed by SUS' Electronic Health Record (EHR) and Practice Management software; documents all services in accordance with SUS policies, OASAS regulations and all relevant federal laws; maintains the strict confidentiality of consumers currently engaged in or formerly a recipient of treatment services.
- Provide individual and group support (Mental Health & Substance Use D/O)
- Provide crisis intervention as appropriate.
- Make community referral recommendations as appropriate
- Utilize best and evidence-based approaches to treatment consistent with SUS's organizational culture; health/mental health integration, Trauma-Informed Care rehabilitation and recovery, culturally responsive care and wellness approaches figure prominently.
- Conduct psychosocial assessments and related assessments.
- Coordinate with community and mental health agencies, substance disorder treatment programs, including Harm reduction support programs and service providers.
- Supportive counseling to agency consumers as needed
- Regular home visits and hospital visits, as necessary
- Maintenance of a strong working relationship with all therapeutic support services to ensure the best quality of care
- Work as a member of the social service team in the development of off-site recreation, socialization, educational and skill building activities with a view towards community inclusion and integration
- Liaison to community-based services and groups including civic, social service government, and business organizations
- Participate in ongoing individual and group supervision, team meetings, case conferences, division meetings and trainings
- Adhere to on call cell phone system to facilitate crisis intervention, emergencies and other situations that require immediate action

I. REQUIREMENTS:
REQUIRED EDUCATION AND EXPERIENCE
- Master’s degree required
- LMSW, LCSW, LMHC, or LCAT required
- 3 years’ experience working with individuals with mental illness, substance use issues and/or a history of homelessness or 3 years’ experience working in the human services field required
- High degree of computer literacy
- Ability to work some late evenings including weekends
- Familiarity with recovery-oriented, evidence-based clinical practice preferred
- Bi-lingual Spanish preferred
