We are seeking a dedicated Independent Facilitator to join our team and provide person centered planning services combined with support to help individuals with disabilities enroll in California's Self Determination Program (SDP). This position includes providing long term coaching with the goal of helping individuals achieve their life goals and find the appropriate supports. This remote position requires reliable transportation as occasional in-person meetings with team members and clients is essential.
Responsibilities:
• Facilitate person-centered planning with individuals and their circle of support, both virtually and in person.
• Assist clients in enrolling in and navigating California’s SDP, including budget development and service coordination.
• Provide ongoing support to individuals, ensuring their plans and services align with their goals.
• Maintain records and tracks client activities in accordance with company policies.
Qualifications:
• Bilingual and bicultural in Spanish is strongly preferred.
• Minimum of 3 years of experience working with individuals with disabilities (preferred).
• Bachelor’s degree in human services, education, or a behavioral health-related field (preferred).
• Ability to work independently in a remote environment with a flexible schedule, including nights and weekends as
needed.
• Valid CA driver's license, reliable vehicle, and current auto insurance.
• Proficient with Microsoft Office 365 and Zoom.
Compensation:
$21-$25 per hour, based on experience. 20-30 hours per week. Partial reimbursement for internet and cell phone provided.
