About the role
The Pear Suite Community Health Worker (CHW) promotes wellness by helping people adopt healthy behaviors. The CHW executes tasks on various community projects and programs and advocates for those who may have limited access to health education, resources, and social services. The Pear Suite CHW utilizes comprehensive assessments, and a range of peer coaching strategies, including Motivational Interviewing and person-centered approaches to care to identify and address Social Determinants of Health needs. The CHW will use the Pear Suite software platform to engage with individuals and the community through in-person, telephonic, and other virtual communication forms, including email, text, and video.
The CHW may spend part of their day remotely (Work From Home) for activities such as telephonic and virtual outreach and documentation. The CHW also spends time in the field to interact with community members, participate in health-related events, and collect data. The CHW may travel to a variety of settings including home, medical/care facilities, and religious or community centers.

What you'll do...
Responsibilities and Duties
Attend regularly scheduled and ad hoc meetings and maintain communication with program team members and supervisor;
Assist in the production of required weekly, monthly, and/or quarterly data collection reports
Maintain outreach activity calendars and logs according to program standards
Assist with program evaluations and updates to assigned program and services
Coordinate care among individuals, communities, and health and social service systems using the Pear Suite platform
Outreach and provide comprehensive education and information through telephonic communication, street outreach, health fairs, community relationships, events, and other approved activities;
Attend appropriate community resource and collaborative meetings and trainings, as assigned
Work in collaboration with other departments
Provide culturally appropriate health and informational resources
Oversee case management and resource access
Advocate for individuals and communities regarding housing, food security, and other Social Determinants of Health needs
Provide coaching and social support, such as nutrition training, recovery services for mental health, substance use disorders, chronic disease management and prevention
Increase individual and community participation in assessments and education to improve health and well-being
Practice cultural humility, knowledge, and awareness of community cultures and values
Able to communicate effectively by relating questions and concerns to the team and promptly responding to team-wide communications

Qualifications

Minimum Qualifications
High school diploma or equivalent. College-level coursework in social sciences, public health, education, or related fields is preferred but not required.
One (1) year experience or more of related work or volunteer experience, involving one or more of the following: health service, community service, language/accessibility service, community organizing, community advocacy, grass-roots mobilization or outreach, and/or education.
Access to internet/Wifi.
Reliable transportation.
Must be bi-lingual Spanish/English.
License
Valid State Driver’s License, preferred.
Community Health Worker certificate, preferred.
