Responsibilities- Develops and implements a nutritional plan based on an assessment of nutritional needs. This plan is included in the patient's/client's plan of care.
- Evaluates and reports results of nutritional assessments appropriately.
- Provides recommendations to the home care personnel team regarding appropriate counseling to individuals and families/caregivers in the home, evaluation and instruction on nutrition principles, dietary plans, food selection and economics, adapting plans to the individual's lifestyle as indicated and re-evaluations of nutritional care through home follow-up care.
- Communicates and documents appropriate dietary history and subjective and objective nutrition care data in the clinical record, including the discharge summary.
- Communicates with care coordinator/case manager regarding patient's/client's progress and treatment goals.
- Maintains confidentiality and safety of patient/client clinical records.
- Complies with the Organization's policies and procedures.
- Performs other duties as assigned.
- Provides in-services as requested by the Executive Director.
- All other duties as assigned.
Education and ExperienceLicense Requirements
- Registered by the Commission of Dietetic Registration.
- Current CPR certification required.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
Additional State Specific Requirements
- NJ, State licensure required.
