Description
This is a great opportunity for seasoned or new Nurse Case Managers! EK Health is looking for a caring, compassionate Field Case Manager (RN) for the Los Angeles, CA area. This position is hybrid role, working from home and travelling as needed to clients, doctors and patients. As a Field Nurse Case Manager at EK Health, no two days are the same. Case referrals come in every day. While you're working from home, you'll complete paperwork, field notes and coordinate medical services. Out in the field, you'll go to sites to be a hands-on Case Manager. Position Specifics: Non-Exempt, Full-time, Standard eight (8) hour workday, Monday-Friday, 8-5 local time (may need to flex based on business needs) Remote with local travel.
Here's a glimpse of what you'll be doing:- Completing in person visits at medical providers, employers, and injured employee's homes
- Performing head-to-toe nursing evaluations to determine needs of patients
- Evaluating clinical status of injured workers and researching options for treatment
- Developing and implementing care plans with patients, physicians, and health care providers
- Arranging for necessary care/services (home care, procedures, medication, equipment, or supplies)
- Preparing comprehensive notes and following up on injured workers, medical providers, claims examiners, and employers
Pay and Perks:- This position starts at $40-$50+ per hour based on experience, education, certifications and location. Additional compensation may be offered with certain certifications (CCM, COHN, COHN-S, CRRN).

Requirements
- Graduate of an accredited school of nursing
- Valid state-appropriate RN license in good standing with no restrictions
- Valid state-appropriate Driver's license in good standing
- 3-5 years' experience as an RN in clinical settings outside of school
- Strong computer skills (MS Word, Excel and internet)
To thrive in this role, you'll need critical thinking skills to ensure treatments are necessary, determining the progress of the care, and modifying care plans to insure the best outcome for the injured worker. You'll also need to be self-directed and motivated. Experience with Workers' Compensation, Occupational Health, Home Health or Field Case Management will assist with your success.
