Job DescriptionPURPOSE OF POSITION: Under the general administration of the Director of Nursing and in collaboration with a physician and accountability to the Medical Director and/or designee, the Adult Health Nurse Practitioner will function in an expanded role in the area of Community Sexual Health. With appropriate medical supervision, s/ he will assume a comprehensive and responsible function in sexual health care for adults aged 18 and older.
DUTIES & RESPONSIBILITIES:1. This position requires working about 20% of the time during late-night or evening shifts at a mobile sexual health clinic.
2. Provides medical services within the scope of approved privileges,
3. Documents all patient contact in the EMR in a timely manner.
4. Assumes responsibility of patients they care for.
5. Collaborates to provide care coordination of activities in providing comprehensive care. (e.g. other healthcare providers, local health departments, social service, nutritionist, dentist,
community based organizations, home care and patient education groups).
6. Provides relevant health education and instruction; counsels patient, responsible for understanding the impact of illness and the principles of health promotion and maintenance.
7. Provides clinical and didactic instruction to professional staff, peers, students and community groups and conducts in-service educational programs, as needed.
8. Participates in departmental meetings and interdisciplinary or community committees, as well as Peer Review Performance Improvement meetings and activities, as required.
9. Follows Gotham, NYC HHC and facility-specific policies and protocols (including but not limited to policies around DNKA, Normal Notification, callouts, dress code, corporate job descriptions.)
10. Meets all compliance standards and requirements (including but not limited to annual physical assessment, fraud and HIPPA trainings, Fire Drill participation.)
11. Ensures that credentials are maintained.
12. Maintains Patient Safety and Infection Control Protocol as per Network and Departmental Policies
13. Upholds environmental safety standards to assure safety of patients and staff
14. Fosters/exercises Service Excellence in daily activities, when dealing with general public and staff
15. Performs administrative, professional and consulting duties as requested by the Chief of Service, Medical Director, or designee.
16. Performs other duties as needed.

Minimum Qualifications1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
2. Current certificate as a Nurse Practitioner issued by the NYSED; and
3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
Additional QualificationsBoard certification is REQUIRED.
Department PreferencesQUALIFICATIONS FOR THE JOB:MACHINES, TOOLS, EQUIPMENT OPERATED: Computer, Printer, Fax, Telephone, Sonogram, NST fetal monitor, speculum, vaginal light, exam table, scale.
QUALIFICATION REQUIREMENTS (see Corporate Job Description)
1) Satisfactory completion of an approved Nurse Practitioner graduate course consisting of classroom and clinical experience and two (2) years of satisfactory advanced clinical nursing
experience.
2) Certified and Licensed as Adult Health Nurse Practitioner by New York State Department of Education
3) Valid NYS License and current registration to practice as a Registered Professional Nurse
4) Maintain a current DEA Certificate.
5) Flexible work schedule is a must as this position requires working evenings, including weekend evenings, for mobile sexual health clinic sessions
SKILLS:Excellent clinical acumen, interpersonal skills, excellent procedural skills, able to navigate EMR.

BenefitsIn addition to base compensation, you may also receive the following based on the Collectively-Bargained Agreement:
• An annual education differential if you currently possess an appropriate degree in nursing or an allied health discipline from an accredited college or university, as listed below:
o Bachelors of Science in Nursing (BSN) or Bachelors in allied health discipline = $900
o Masters of Science in Nursing (MSN) or Masters in allied health discipline = $1,200
o Doctorate in Education, Nursing Practice or PhD in Nursing (DNP) = $2,500
• An annual certification differential of $1,878 if you currently hold an appropriate certification that is nationally recognized in the discipline or area of specialization for which you are employed.
• An annual nursing experience differential ranging from $1,097 up to $9,100 for up to 10 years of experience if you have outside nursing experience as a Nurse Practitioner.
