Overview
The Manager of Patient Case Management administers the day to day operations of the Social Workers and the Discharge Specialist in the Patient Case Management Department, works collaboratively with the Patient Case Management leadership and the Patient Case Management Physician Advisor to carry out department goals. Responsible for implementing and maintaining daily social work departmental operations with consistent standards of practice. Maintains clinical competency and serves as clinical resource to staff of the Patient Case Management Department and to the physician and leadership of WakeMed. Works closely with both WakeMed physician and leadership to support patient progression and appropriate resource utilization throughout the system. Collaborates with physicians, nursing, members of the interdisciplinary team, PFS, Patient Access and other departments, to ensure that services are being provided in a timely/quality manner and act as a resource when resolving barriers. Analyze data to improve the effectiveness of the Social Work services provided by Patient Case Management program.
