Advocate for patients in end-of-life phase. Educate patients and families about end-of-life symptoms and options to assist them in making informed decisions. Provide individuals and families with the psychosocial support needed to cope with terminal illnesses. Refer family to community resources to assist in recovery from emotional distress and to provide access to services such as financial assistance and legal aid. Organize support groups or counsel family members to assist them in understanding, dealing with and supporting the patient. Provide patients with information and counseling. Monitor patient progress according to goals in care plan. Collaborate with other professionals to evaluate patients’ condition and to assess patient needs. Utilize consultation data and counseling and social work experience to plan and coordinate patient care, following through to ensure service efficacy. Monitor, evaluate, and record patient condition and care plan.
