Esperanza Community Housing Corporation is a 35-year non-profit organization working to achieve comprehensive and long-term community development in the Figueroa Corridor area of South Central Los Angeles. Project Description: Esperanza-Lanterman Regional Center Project
Esperanza has partnered with the Frank D. Lanterman Regional Center to develop a one of a kind project with the aim of improving family’s access and utilization of services available services that exist within the regional center as well as in their community. The focus of the project is to improve the utilization of services for hard-to-reach clients as well as improve organizational systems to improve outreach activities for underserved clients. The Promotor Outreach Project will integrate Promotores to serve as liaisons that will help increase the knowledge, understanding and utilization of services in addition to strengthen the relationship between Lanterman and the community it serves.
The Community Health Promotor will work in collaboration with the Lanterman Regional Center Service Coordinators to meet project outcomes and lead the home visits for 26 underserved Latino Lanterman cases. Promotor will lead direct communication with Lanterman Service Coordinators. The Promotor is responsible for delivering monthly and quarterly goals, reports, training, coordination of field activities and liaison with staff of Lanterman. The Promotor will also be responsible for integrating Lanterman Project with the larger health mission and activities of the organization and be assigned other organizational duties as required.
Duties and Responsibilities:
- Provide a minimum of 1 home visits on a monthly basis to 26 Lanterman Regional Center clients.
- Identify needed services based on family’s current situation via home visitations using Esperanza/Lanterman Project protocols and program data collection tools.
- Write and submit monthly reports to Director of Health Programs focusing on care plans and tracking form.
- Support with quarterly reports via collection of necessary data to report on achievement and evaluation of goals and objectives.
- Maintain regular data entry on cases available on organizational server.
- Participate in weekly/bi-weekly/monthly case review as well as additional case review as needed.
- Coordinate, and monitor program evaluation activities
- Represent Esperanza on relevant coalitions/collaboratives.
- Represent agency at key events related to the Lanterman project or Esperanza Health Programs as required.
- Collaborate with all Esperanza Health Program staff to meet aligned project goals.
- Assist with project coordination for other programming and projects for Esperanza as needed.
Qualifications, Education and Experience
- Understand and support the Esperanza’s Vision, Mission, & Values Statement
- Graduate of Esperanza’s Community Health Promoter Training required
- Bilingual in Spanish/English required
- Experience working in a multicultural setting and sensitive to multicultural issues
- Possess good interpersonal relationship skills
- Shows compassion and empathy, warmth and caring in relating to children and families and larger community.
- Creates a feeling of confidence in patients/families, so they will comfortably express their questions and concerns
- Demonstrates excellent communication skills necessary for public speaking and family education
- Self-starter, able to take initiative and make intuitive decisions
- Able to work in an autonomous setting, yet function as part of a larger team
- Must be physically able to lift, carry and handle equipment, supplies and other
program material
- Ability to utilize email, Microsoft Office systems for project reporting and daily communication.
