Job Summary:
We are currently seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a crucial role in providing support and guidance to individuals in need. The ideal candidate will have a strong background in mentoring, crisis intervention, and addiction counseling.
Job Description:
The role of the Case Manager is to provide therapeutic interventions for client’s coming into the program for detox and assisting with the client’s transition to aftercare. The Case Manager is responsible for conducting biopsychosocial assessments, discharge planning, treatment groups, communicating to family/support system regarding client’s care, and providing clinical documentation for the services provided. The Case Manager will coordinate treatment plans conducive to the client’s treatment goals and help with providing outside resources when necessary. The Case Manager’s role within the treatment team is to communicate the clinical needs, progress, and transition planning updates to the multidisciplinary team in a manner that supports client’s dignity and promotes treatment team cohesion. At times the Case Manager will be asked to provide clinical support to floor staff if management is off site.
Job Responsibilities:
- conduct biopsychosocial assessment
- communicate to family/support system
- work closely with Utilization Review Analyst on clinical documentation
- coordinate treatment plans conducive to the client’s treatment goals
- able to articulate clinical needs, progress and transition planning to multidisciplinary team in a manner that supports client’s dignity and promote treatment team cohesion
- complete intake documentation within 24 hours period of service provided
- team collaboration with all staff (nurse and clinical teams) to address crisis issues, assess need, and/or referral of services
- assist Clinical Director/Primary Therapist in the coordination of services to client; treatment plans; monitor and implement opportunities for new program development
- responsible for discharge planning (planning and implementation of integrating aftercare services within the community, provide resources, and linkage)
- facilitate and run group therapy
- assist clients in creating a comprehensive discharge plan that includes community referrals to support ongoing sobriety.
- crisis intervention, including emotional support and de-escalation of crisis situations
- brief individual and group rehabilitation treatment, and support system counseling as needed
- assists Clinical Director in the coordination of services to clients
- remain in compliance with HIPAA
- respond to emergencies and initiates action as required
- attends and participates in staff meetings and trainings
- other duties as assigned
Minimum Requirements and Qualifications:
- Licensed or Certified with an approved Department of Health Care Services certifying agency to provide chemical dependency or other counseling services: CADC-I, CADC-II (required)
AND
- Minimum 2 year’s experience working with individuals in chemical dependency and abuse
- Minimum 2 year’s experience providing group, individual and family/significant other education and group therapy (preferred)
- Knowledge of FMLA, EDD and STD (required)
- Experience and education in chemical and alcohol dependency and abuse (required)
- Ability to communicate and work effectively with people from various backgrounds (required)
- Current CPR and First Aid Certification (required)
- Ability to communicate and work effectively with people from various backgrounds (required)
- Commitment to the philosophy of recovery and continuity of the program and of consideration and respect for clients
- ACTIVE Driver’s License and insurability on company's commercial policy (required)
Job Type: Full-time
Pay: $22.00 - $24.00 per hour
