Salary Range (Annualized) $70,720- $76,960
Monitors and coordinates the delivery of health care services for individuals with developmental disabilities who are attending People Inc. Day Programs and living in community based homes. Supervises the adequate completion of delegated nursing tasks to direct care staff and LPN’s. Provides / participates in Agency training.
ESSENTIAL FUNCTIONS / RESPONSIBILITIES
- Monitors and coordinates the delivery of preventive and routine health care for individuals with developmental disabilities in both the day program and residential settings
- Provides Agency training as required
- Coordinates and provides health services and training on those services for staff
- Oversees delegated health related tasks to Direct Care Staff and LPN’s
- Provides medication training and certification to direct care staff per regulations and Agency policy
- Adheres to and ensures facility compliance with all physicians, NP, PA, etc. orders
- Based upon the independent judgment of the RN, the RN has the authority to suspend and/or revoke the responsibility of a Direct Care Staff or LPN to perform or complete any delegated task
- Based upon the independent judgment of the RN, the RN as the authority to suspend and/or revoke medication certifications and medication administration of the direct care staff or LPN.
- Completes a physical health and development on individuals every 6 months in day programs
- In coordination with program managers in day programs, ensures safeguards are updated as needed
- Within each month, completes a monthly breast exam on female clients if specifically ordered at day program by PMD
- Coordinates medical program needs with community services, medical providers and other related agencies
- Ensures adequate scheduling and completion of health related exams, appointments, laboratory / diagnostic testing as ordered or required
- Monitors medication administration in the facility
- Completes a thorough Annual Medication review (Non-ICF) or ensures a pharmacist completes a Quarterly Medication Review (ICF)
- For ICF’s, completes quarterly nursing reviews for each person receiving services
- Within each year, updates the Plans of Nursing Services and ensures review with staff
- Within each month, completes a monthly breast exam on female clients, if ordered by a physician and / or provides training to individual
- Coordinates and provides health services / training for staff
- Oversees delegated health related tasks to Direct Care Staff and LPN’s
- Provides medication training and certification to direct care staff per regulations and Agency policy
- Participates as an active member of the Interdisciplinary Team
- Maintains compliance with all applicable State / Federal regulations and Agency policy and procedure
- Maintains communication with LPN (as applicable), Program Manager, Program Administration, Service Coordinator and other staff as necessary
- Full time residential RN’s will follow a rotation schedule for emergency staff training on Saturdays and Sundays
- Complies with all Agency policy and procedures
- Other duties as assigned
MINIMUM QUALIFICATIONS
- Current NYS license to practice as a Registered Nurse
- NYS Drivers License that meets Agency policy
- 1 year of related experience
- Ability to meet physical requirements of the position when performing the following:
- Standing
- Walking
- Sitting
- Bending
- Stooping
- Squatting
- Kneeling
- Climbing
- Lifting requirement of 35 lbs
- Ability to move, reposition, and place individuals in pieces of equipment
- Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention (SCIP-R) as per Agency guidelines
