As a case manger you will provide resources, education and support to clients. As well as develop and implement individualized care plans.
Primary Responsibilities:
- Provide education and support families
- Coordinate services within and outside of the clinic
- Help clients access and utilize community resources
- Act as a liaison between the client and service providers

Required Qualifications:
- Bachelor’s or Master’s degree in social work, psychology or a related human services field
- Experience with case management or other mental health related experience
- Proven solid verbal and written communication skills
Preferred Qualifications:
- Experience with couples and family counseling
- Experience with children or adolescents
- Experience with health psychology
- Competent with a wide range of clinical requisitions
