Job description Summary:
If you like helping people, this job is for you. You will implement ideas and help your clients become their ultimate best by assisting them to reach their goals. Daily activities will enhance the development of their skills. Each consumer's goals and objectives are different. On one day you may be taking them to the gym to increase their health and wellness goal or on another day, you will be assisting them as they study to take a test for school. You will need to maintain all assigned documentation and data regarding their progress. As the educator you will sometimes need to provide personal care for them. Punctuality and empathy will be an important skill that is needed to carry out these responsibilities. Monthly mastermind meetings to collaborate with your peers and case manager will be held to ensure you are supported with the resources necessary for an excellent outcome of goals.
Description:
Under general direction of the Case Manager assists in coordinating and planning schedules of daily activities to facilitate learning experiences
Serves as an additional liaison with the consumer’s family and other agencies
Position Requirements:
Valid CA Driver’s License
Safe, reliable vehicle with current registration and insurance
Recent DMV print-out with good driving record
Ability to lift between 50 to 75 lbs. depending on individuals served and pass post-offer physical
CPR and First Aid training within 14 business days of start date
Read, speak, write English
Ability to work independently with or without direct supervision
Exercise sound judgment at all times
Ability to effectively and communicate in writing, i.e. telephone, email, reports, etc.
Desirable Qualifications:
6 months of volunteer or paid work experience of persons with disabilities
Ability to conform to work schedule that may be split
Bilingual in Spanish and or Chinese or Mandarin
High School graduate or higher education preferred
Ability to communicate effectively written and orally
Experience in providing community based direct services for individuals with disabilities
Ability to work independently without direct supervision
Ability to creatively develop an individual program of services to meet consumer needs
Requirements

Life Scan Background Check
High School Diploma
Ability to lift between 50 and 75 pounds depending upon individuals served
Have CPR and First Aid Certification upon 1 week of becoming employed
