Human Services, a department of the City of Seattle, funds and operates programs and services that meet the basic needs of the most vulnerable people our community - families and individuals with low incomes, children, domestic violence and sexual assault victims, homeless people, seniors, and persons with disabilities. We invest in programs that help people gain independence and success.
Aging and Disability Services division (ADS) manages the continuum of care that supports older individuals and those living with a disability, and their families. The mission of Aging and Disability Services is to develop a community that promotes quality of life, independence and choice for older people and adults with disabilities in King County. Our goal is to promote healthy aging, ensuring that our communities are great places for people to grow up and grow old.
As the state-designated local Area Agency on Aging (AAA) for Seattle and King County, ADS work in
partnership with King County to:
- Improve the health and quality of life for older adults and adults with disabilities,
- Connect seniors and adults with disabilities, and
- Provide help and support for caregivers.
We are seeking two new creative and collaborative team members to join and support our team development with a focus on supporting unpaid caregivers and client services. This position will complete TCARE® assessments and authorize respite services to aging and disabled clients in the Seattle and King County area. We are currently looking for two Program Intake Representatives for our Aging and Disability Services Division in our Seattle office with caseload covering all areas of King Co. This position will report to the Human Services Program Supervisor, Sr. of the Case Management Program and be part of the Family Caregiver Support Program and MAC/TSOA team.
This position is required to be in the office/field a minimum of three days a week. A valid Washington State Driver License is required.
Job ResponsibilitiesJob Responsibilities (include but not limited to):Interviewing clients, evaluate their needs and determine eligibility for respite services through the Family Caregiver Support Program
- Assess client eligibility and need for respite services using the TCARE® assessment tool
- Educate and assist clients in selecting other community services
- Conduct outreach activities which may include presenting information to community groups
- Monitor caseload to track when clients need 6-month rescreening, reassessment or become ineligible
- Prepare and send correspondence on behalf of clients and to clients
- Maintain and update records and complete forms for each client
- Provide information and referrals for services available through other agencies
- Schedule and coordinate setting up initial and ongoing respite services
- Create and manage monthly authorizations
- Work effectively within diverse workgroups and populations with proven ability to collaborate with community based and governmental agencies
- Understanding of race and social justice principals and impacts of systemic racism and oppression on communities served by ADS and HSD
QualificationsRequires Two years of college education in a social science, education or related field and two years of experience interviewing or counseling clients, or in community outreach, preferably in a social service setting (or a combination of education and/or training and/or experience which provides an equivalent background required to perform the work of the class).
Certification/License: Current State of Washington driver's license in good standing, or evidence of equivalent mobility.
Desired Qualifications:- Experience working in TCARE® and GetCare systems, although not a requirement
- TCARE® certification
- Detail oriented, ability to efficiently manage a high caseload of 80 - 110 cases
- Responsive to client and community partner inquires timely
- Work well independently
- Strong commitment to Race and Social Justice principles.
