We are seeking a dedicated Behavioral Health Professional to become a member of our diverse holistic care team! In this role, you will work as a member of the primary care team providing clinical consultation, assessment and brief intervention in the medical clinic setting.
Key Responsibilities:
- Provide comprehensive assessment and diagnosis of behavioral health clients.
- Provide assessment, consultation, and brief intervention for psychological/ psychiatric problems and/or disorders.
- Assist patients in identifying behaviors which interfere with general wellness and provide brief interventions to aid in behavior change.
- Provide effective treatment planning and assisting clients in successfully achieving goals.
- Provide behavioral health triage, evaluate crisis situations, and apply appropriate interventions.
- Actively participate in meetings that support HealthFirst Bluegrass’s integrated health care model to provide comprehensive care for patients/ clients.
- Provide trainings for staff and/or families on behavioral health issues as needed.
- Collaborate on clinical/ case supervision with both behavioral health and primary care team members.
- Provide supervision to students rotating with HealthFirst Bluegrass.
- Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration.
- Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
- Teach patients, families, and staff care, prevention, and treatment enhancement techniques.
- Monitor the site’s behavioral health program, identifying problems related to patient services, and making recommendations for improvement.
- Work with third party vendors and community resources on behalf of the organization and/ or clients to ensure needs are met.
- Must comply with HIPAA, HITECH, and Part II regulations.
- Other duties as assigned.
Qualifications:
· Must be licensed to practice independently in the state of Kentucky (LCSW, PhD, PsyD, or equivalent)
· OR
· A Certified Social Worker with relevant experience.
· Must maintain compliance with licensure board continuing education requirements.
