We are seeking a compassionate Pharmacist to join our team and support our goal of providing equitable care to all patients.
Key Responsibilities:
- Review Physician and Clinician prescription orders.
- Check Pharmacy Assistants’ work for proper preparation of prescription including correct drug, directions, Physician, and patient name, refills and payor source
- Record information that is necessary to conform with State and Federal laws and regulations
- Instruct the patient about proper use of medications (or supervise the Pharmacy Assistant in this activity)
- Determine the appropriate course of action if a drug-drug or food-drug interaction occurs
- Interact with Physicians and other clinical staff as needed to answer questions regarding prescription information
- Participate in the evaluation and treatment of patients with selected diagnosed chronic illnesses per protocol
- Evaluate drug therapy regimens and make appropriate changes after conferring with clinical staff
- Edit and distribute the Pharmacy monthly newsletter if applicable
- Assist in maintaining and updating the drug formulary every six months if applicable
- Review patient records to determine the appropriate cause of action when a patient has no refills on a medication
- Interact with drug manufacturer representatives if necessary
- Place orders with selected pharmaceutical manufacturers and for all Schedule 2 pharmaceuticals if authorized to do so by DEA.
- Keep a running inventory of all Schedule 2 pharmaceuticals
- Provide back up in the absence of the Chief Pharmacist
- Supervise the actions of the Pharmacy Assistants while on duty
- Collect and organize daily prescription logs on a monthly basis
- Distribute reports to selected program areas when requested
Qualifications:
- · 5 year Bachelor of Science in Pharmacy and 5 years of ambulatory care Pharmacy practice experience.
- · 6 year PharmD, PharmD/MBA, or Pharm/MPI with 1 year of practice experience.
- · Must be licensed to practice Pharmacy or eligible for licensure in the state of Kentucky.
- License/Certification:
Must be licensed to practice Pharmacy or eligible for licensure in the state of Kentucky. Current CPR is required and must be maintained throughout the duration of this position.
