The job is across Ventura County - Simi Valley, Thousand Oaks, Camarillo, Oxnard, Ventura (depending on location of the client)
1) Help/support/teach individuals served to develop skills and confidence to live independently, such as:
Independent Living Skills
• Household management - including laundry, cleaning
• Financial management - budgeting, banking, bill payments
• Safety & Survival Skills
• Health & Wellness Needs - advocacy with healthcare providers, nutrition, meal planning, grocery shopping & meal preparation.
• Use of community resources, including public transportation
• Community integration & social support - including companionship
• Social skills
• Advocacy & mentorship
Tailored Day Services
• Support the individual in leading meaningful lives in the community, including
• Post secondary education – supporting in enrollment in courses, accessing disabled student services or any other accommodation, acclimating to classes, completing coursework and related activities
• Employment – job/internship search, resume review or writing, mock interviews, job application
• Community Integration & Personal Development – safety & survival training, socialization, mobility training, transportation education, social activities
2) Maintain proper documentation relating to the individuals
3) Communicate & collaborate with Individual’s family, supervisors and co-workers
4) Must be Bi-lingual to communicate with individual served
5) Performs other duties as required.
