Under the direction of an RN, you will assist with the monitoring and coordination of the delivery of healthcare services with a manageable caseload for people with developmental disabilities who are in a structured day program or living in community base homes. As a human services agency we are seeking candidates with strong leadership skills, compassion and the ability to work in a collaborative environment.
In a Residential setting, assists the RN to provide medical case management for individuals with developmental disabilities who are living in community-based homes.
In a Family Care setting, assists the RN in working with people living with certified caregivers and supporting individuals in the caregivers' home.
In a Day Service setting, assist the RN to ensure that nursing services are provided for those individuals needing services during the hours of program attendance.
In a Respite setting, assists the RN to provide nursing oversight to people respiting and staff supporting them in a respite group home setting.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES:
• Assists with monitoring and coordinating the delivery of preventive routine health care, therapies and programs for individuals with developmental disabilities
• Assists with ensuring adequate completion of health related exams, appointments, laboratory/diagnostic testing as ordered or required.
• Assists with the monitoring of medication administration in the facility.
• Administers medications, treatments and supplements as assigned.
• Assists the RN with providing health services policy training for staff.
• Attends medical appointments at the direction of the RN.
• Participates as an active member of the Interdisciplinary Team.
• Maintains compliance with and assists with ensuring facility compliance with all Physician, NP, PA, etc. orders, applicable State/Federal regulations, and agency policy and procedure.
MINIMUM QUALIFICATIONS:
• Current NYS licensure to practice as a Licensed Practical Nurse
•Valid Driver's License that meets agency policy
• Ability to meet physical requirements of the position when performing the following: Standing, walking, sitting, bending, stooping, squatting, kneeling and climbing
• Lifting requirements of 35lbs
• Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention (SCIP-R) certification as per regulatory guidelines
• CPR certification required
